Filing a Name Change Petition


Petitions for name change are filed with the District Court and require two forms: Petition for Changing Name and Order for Changing Name. Please complete the interview process for a name change petition online. (This is an electronic filing or view the Name Change Information Packet). The forms may be picked up at:

If the name change request is for a minor child, each parent must sign an affidavit indicating that they both agree and consent to the change.

Note: The Name Change Information Packet is in a .PDF format. In order to view the file, you must have Adobe Acrobat Reader installed on your system. You can download the Adobe Acrobat Reader software at no cost; OR complete the interview process for a name change petition online. (This is an electronic filing.)

After you submit the petition online, you will be given a confirmation number. You must appear at the Public counter (Room 239 at the CCB) within 72 hours with your confirmation number to pay the filing fees outlined below and receive a copy of your petition. A court date will be scheduled, which is usually the same day as you appear at the counter. Your Petition will not be processed until you appear at our public counter.

Fees


A $179 filing fee is required. The fee for each additional family member is $89. These additional costs are for three certified copies and the statutory transmittal fee to the Auditor's Office for recording fees. New name change cases may be processed the same day if submitted between:
  • Monday - Thursday
    9 a.m. - 11:30 a.m.
    1 p.m. - 3 p.m.
Name changes are accepted until 3 p.m. Monday through Friday at the Court's Operations Division (CCB).
